The Rural Health Transformation Program (RHTP) is a new federal initiative introduced through the One Big Beautiful Bill Act (OBBBA) to strengthen healthcare in rural communities. It provides funding to help hospitals, clinics, and providers modernize operations, improve patient care, and stay compliant with evolving federal requirements.
The program sets aside $50 billion over five years (FY 2026–2030) with $10 billion per year distributed to states. Half will be shared equally among eligible states, while the other half will be allocated based on need.

Funding for the Rural Health Transformation Program (RHTP) will be distributed through state-level applications. Each state must submit a one-time Rural Health Transformation Plan to CMS by December 31, 2025, outlining how funds will be used (CMS). Applications are expected to be released in mid-September 2025, with a submission window closing in early November, and awards announced by the end of the year.
